Swollen ankles at the end of the day. Aching calves that throb once you finally lie down. A lower back that never quite relaxes, even on a good mattress. These are common reasons people start searching for whether sleeping with feet elevated helps, and if so, how to do it without creating new problems.
For most healthy adults, sleeping with the feet elevated 6 to 12 inches above heart level can improve circulation, reduce overnight swelling, and take pressure off the lower back. But the details matter. Height, angle, duration, and the surface you’re sleeping on all change whether leg elevation helps or simply trades one discomfort for another.

Gravity is the issue. When you stand or sit for long stretches, blood and lymphatic fluid have to work against gravity to return from your legs to your heart. Over the course of a day, fluid accumulates in the lower extremities, vein valves strain, and the small pressures add up. Raising your legs above heart level reverses that flow.
The established benefits fall into three categories:

For people with varicose veins specifically, 15 to 20 minutes of elevation three to four times daily is a common clinical recommendation. Chronic venous insufficiency (CVI) affects roughly 25 to 40 percent of women and 10 to 20 percent of men, and about one in three adults over 45 develops some form of vein disease. CVI symptoms such as heaviness, aching, and night cramps often worsen in the evening, exactly when you’re trying to fall asleep. Elevating the legs above heart level helps drain pooled blood and can quiet those symptoms overnight.
Late-pregnancy edema is extremely common. The Merck Manual recommends lying in the left lateral decubitus position, elevating the lower extremities, using compression stockings, and wearing loose clothing to manage physiologic edema. Pregnancy also raises DVT risk roughly fivefold, which makes circulation-supporting practices more relevant, not less. Always clear new symptoms with your OB, since swelling can also signal preeclampsia.
Leg elevation after lower-body surgery or injury is a long-standing clinical recommendation. Raising the affected leg reduces swelling, fluid buildup, and inflammation, and supports faster recovery. Your surgeon or physical therapist will give specific instructions for duration and height.
The human spine has a natural S-shape. Lying completely flat flattens the lumbar curve and can concentrate pressure in the lower back. Placing support under the knees rotates the pelvis slightly and lets the lumbar spine settle into a neutral position. Many back sleepers find this single change more comfortable than any amount of mattress shopping.
Because elevation supports lymphatic drainage, it’s frequently part of at-home management for lymphedema alongside compression and manual drainage techniques prescribed by a clinician.
The sweet spot for most people is 6 to 12 inches above heart level. That’s high enough to reverse gravity’s effect on venous return without hyperextending the knees or straining the hip flexors.
Two details matter more than the exact inch count:
A wedge or leg-elevation pillow placed under the calves (not directly under the knees) with a small cushion supporting the natural knee bend is the cleanest setup. Regular bedroom pillows tend to slip, deflate, and create the wrong angle, which is why dedicated leg-elevation pillows or an adjustable base produce more consistent results.
The zero gravity position was originally developed by NASA to distribute astronauts’ body weight evenly during launch. On an adjustable bed, it translates into raising both the head and legs so that the knees sit roughly level with the chest and the legs are at about heart level.

It’s not for everyone. Stomach sleepers can’t use it comfortably, and side sleepers sometimes find it awkward compared to a flat surface with a pillow between the knees.

For back sleepers primarily looking for lumbar relief rather than full circulation support, a medium-firm pillow under the knees rotates the pelvis and reduces lower back pressure.
